Through a generous grant via the United Health Foundation and in cooperation with KC Care Clinic, CHC of Wyandotte County’s Community Health Worker initiative seeks to improve health across Wyandotte County. From time to time, we want to showcase some of the success stories from our health workers: 

“Referred to me by NBCCDC,  a patient was looking for dental treatment for himself and five children, as well as a primary care provider. Both children and patient have Medicaid and no regular access to a vehicle. I looked up dental providers for the children in the same zip code. My patient came to the office and called dental providers for children with me, and they eventually made appointment. I referred the patient to Swope Health Services for primary care and dental treatment, and the patient stated they would be going there soon.”
