Through a generous grant via the United Health Foundation and in cooperation with KC Care Clinic, CHC of Wyandotte County’s Community Health Worker initiative seeks to improve health across Wyandotte County. From time to time, we want to showcase some of the success stories from our health workers:

“The patient was referred to me from El Centro.  My patient was having a lot pain in the knee and legs. He did not have a Primary Care Physician established and was not insured. I went with the patient to Duchesne Clinic to qualify for services. He got accepted and now has a PCP established. Patient went for couple of visits already, and for lab works at the Clinic. The clinic was able to prescribe some medication for the pain.

The patient was very happy and feels relived for having a doctor. Even better, he does not need to go to the emergency room for his knee and leg pain.”
